We are seeking a Post 16 Transition Coordinator, to work with some of the most vulnerable young people across Herefordshire as they enter their first steps as adulthood.

Working with the Heads of Centre from The Brookfield School, the Arrow Group and St David’s PRU, you will help to ensure our young people continue on their educational journey as confident, resilient, emotionally literate, ambitious and tolerant young people who are effective communicators in wider society.

You will have experience of working in the SEMH setting at either KS4 and/ or Post 16 and will have a genuine passion for working with children with additional needs. You will be dedicated to working with families, championing vulnerable children and will have a working knowledge of SEN in terms of practice and legislative frameworks. You will take on a facilitation role, working with wider employers, training providers and colleges to ensure that our Y11 leavers stay in education or training.

We want to interview candidates who can evidence that they have experience of closing the gaps in learning for pupils who are experiencing challenge, particularly in regulating their behaviours. Importantly, you are someone who is resilient, innovative and has the type of enthusiasm that is infectious, so that you bring about change in a positive way. You are also sensitive, respectfully challenging, patient and kind: just the type of person our students, pupils and community can relate to.
